What is PRIME?
PRIME - Partnerships in International Medical Education is an organisation based in UK that is becoming an international network of volunteer teachers who make trips to help with education programs for health workers. PRIME also stands for an approach to medical education putting God into the curriculum in a way that will not be offensive to people of different faith backgrounds.
 
What is in the name "PRIME Project of HealthServe Australia"? 
There has been much discussion. The title strongly relates us as part of PRIME as an evolving international organisation, but indicates that HSA is the legal entity in Australia that will provide the accountability for tax deduction etc. We will make good use of the office support provided by HSA and in time contribute towards that.

What is the structure of PRIME Project of HSA?
PRIME Project has approved by HSA board in May 2010. The steering committee/ officers are  Carolyn Russell (trainer), Michael Burke (project officer) and Owen Lewis (coordinator). Carolyn will use her experience and talents to train the trainers in PRIME concepts and methods. Michael, as Project Officer of HSA will guide the organisational development and wear a hat for possible future trips to Africa. Owen will coordinate the mailing list, newsletter and recruitment of volunteer teachers as well as wearing the India hat. One or two other people on our list will be invited onto the steering committee that will function by Skype and email connection. 

What are the criteria for tutors?

Potential teachers may have heard about PRIME Project of HSA through various channels including subscription to newsletters through the HSA web site. Would be visiting teachers will apply indicating their skills and availability. They will submit their CV and supply names of 2 referees - one professional and one church related. The process for selection will be based on that use by PRIME Int. Preference will be given to those with teaching experience and developing country experience, but many candidates may not have both. Two PRIME Project of HSA persons and one PRIME Int. person will review applications. Teachers will understand that HealthServe Australia is a Christian organisation but will not be required to sign the faith declaration, as do those who choose to become members of HSA. During the trips of PRIME Project of HSA, many of the visiting teachers will engage local participants and teachers in conversation about the spiritual aspects of health, but this is not a requirement. 

What is happening about the trips to India?
The opportunity to assist the Christian Medical College Vellore Department of Distance Education continues to be a solid invitation. The idea is that GPs ideally with experience working in developing countries and also teaching experience, will be recruited to act as 'external coordinators' of 'contact programs'. In the 2 year PGDFM distance course there are 3x two week blocks when trainees go to regional centres for the contact programs. The Indian government National Rural Health Mission is going to pay for the training of government doctors in 8 poorer states. Dr Vinod Shah, the architect of the PGDFM has now visited the health secretaries in those states and given them a deadline of the end of October to complete formalities. If that process is successful the government PGDFM program will start in January 2010, which means the first contact program will take place in March 2010.

Recruitment and selection for Indian contact programs
Owen Lewis will lead the team. PRIME Project of HSA requests that potential visiting teachers submit their CV with 2 referees. Many of you have done this already. The applications will be reviewed by 2 PPHSA and 1 PRIME (UK) person. We will know the dates of the three contact programs for the batch of trainees. It would be ideal if visiting teachers can commit to three trips so that there is continuity, but this is not essential. 

Team arrangements for trips to India
Once the team has been formed, checklists for finance, travel arrangements and other preparations will be distributed. It is envisaged that team members will raise their own finance for the trip and arrange round figure donations to HSA to obtain tax deduction if needed. Team members will book their own tickets, liaising with HSA.  CMC Vellore will give a daily allowance for International Consultants that makes a significant contribution to the costs involved.  If visiting teachers go in pairs the daily allowance for one person is shared.  The idea of going in pairs would make cross-cultural new experience, a bit easier but the tasks of external coordinator can be done by one person. A lot depends on the capabilities of the local coordinator - a doctor of the hospital where the regional training is to take place.  Team members will most likely be from different Australian states and possibly from UK, so we will not meet together until we arrive in Vellore (or possibly Delhi) for a two day orientation program. We will then disperse to the eight contact program sites in the different Indian states. 

Contact Programs of the PGDFM course
In the current open seat PGDFM course, contact programs have been going on for the past three years, with CMC Vellore staff and a few others in the external coordinator role. So there is already a pattern. Morning sessions are locally arranged whereas afternoons are video linked across India. The morning program is finalised by the local and external coordinators via email. The local coordinator arranges local specialists to teach in the morning programs. Many will choose to give didactic PowerPoint presentations but these should not duplicate afternoon topics. The challenge for coordinators is to promote interactive educational methods - e.g. cased based (clinic, wards) balanced with skill oriented sessions. While there has been significant freedom and variation of content and style in the morning sessions, it is recognised that visiting teachers may need a more defined role, more structure and more in the way of pre-prepared materials. When I was external coordinator in Chennai my local counterpart had arranged a very busy program involving local consultants, where as in Calcutta, Bruce Hayes found himself doing most of the teaching. I will place sample programs on the HSA web site. In the coming six months we will firm up the expectations so that external coordinators can go to India with adequate preparation.

Nov 2009   Musculo-skeletal teaching trip to Vellore
Norm Broadhurst and Chris Clohesy of South Australia returned after 10 days in Vellore in early November 2009. Norm writes:
It was a privilege to be invited to teach several young Vellore doctors
doing their general practice training how to examine and treat dysfunction
of the vertebral column. Four days were spent teaching 10-15 general
practitioners how to examine and treat dysfunction of the vertebral column.
There was a noticeable eagerness to improve skills in this area. In future
it might be better to have access to patients. The Dept. of General
Practice at Vellore was left with a DVD and numerous hand-out sheets, the
latter of which can be copied for wider use for patients. It seems all saw
the teaching as a worthwhile endeavour and enjoyable. Yours sincerely, Norm

May 2009:
Election delays decisions that affect the trips to India

We are waiting for Indian election to finish so that state departments can get back to work and make decisions about the PGDFM
 
We now have about 20 doctors willing to go on trips to India to help with the teaching. The problem is that we do not know the dates for the "contact programs" for the PGDFM for the government doctors.
 
Australian Leadership Award Fellowship Program
From April 22 - June 16, four Indian and 3 Nepali GP teachers are in Melbourne hosted by Nossal Institute for Global Health and coordinated by Owen Lewis. They are visiting GP training activities - the departments of general practice Melbourne and Monash, the RACGP, Regional Training Programs - Victorian Medical Alliance, Bogong RTP in Shepparton. The fellows include Dr Vinod Shah, Director of Distance Education at CMC Vellore, also Dr Sunil Abraham CMC V, Dr Thomas Abraham (Jiwan Jyoti Christian Hospital, Robertsganj) and Dr Immanuel Rajamani (Christian Fellowship Hospital, Oddanchatram). They are all involved in running the PGDFM.

Weekend conference in "Healthcare Teaching Opportunities at Home and Overseas"

The conference took place over Saturday 4th - Sunday 5th April 2009 in Brisbane. It was attended by more than 25 people. Dr John Geater MBE the Director of PRIME International
(www.prime-international.org.uk)  inspired us with his visionary approach. Negotiations are under way to form a PRIME Australia Chapter under the governence and legal protection of HSA, but with very strong links with PRIME International, sharing procedures and communication pathways.
 
It is very likely that the trips to India will come under the PRIME banner.
  
 
 
 
 
 
March 2009: GPs needed for teaching trips to India
 
 
 
Until recently less than one hundred vocationally trained GPs were registered in the whole of India, while there are 250,000 GPs who have not had post graduate training.
 
 
 
To improve primary healthcare in India, CMC ( Christian Medical College) Vellore in 2007 established the Post Graduate Diploma of Family Medicine by distance education for medical graduates. This course runs for two years and each year currently contains a cohort of two hundred graduates. There are contact programs for groups of 15-20 GPs that are run simultaneously with video hook-up all over India. For each group there is a local and visiting coordinator. Looking at the success of this course, the Indian Government has asked CMC to increase the numbers it is training. Agreement has been made with the Ministry of Health and Family Welfare: National Rural Health Mission http://mohfw.nic.in/NRHM.htm However, as health is a state matter individual states are in the process of making agreement for implemation of the program with CMC Vellore. The following states have been invited to be involved:

  • Madhya Pradesh
  • Bihar
  • Orissa
  • Jharkhand
  • Chhatisgarh
  • Uttaranchal
  • Assam
 
 
 
CMC Vellore has requested help from Australia in recruiting qualified GPs to be visiting coordinators. The curriculum and teaching program is tightly structured. The role of visiting coordinator includes some didactic teaching using pre-prepared PowerPoint etc. and running interactive sessions with and without video link. Although the need is for GPs, other specialists may be able to help in later programs, once it is clearer about the expectations and the specifics of what is being taught in each contact program.  
 
 
 
 
 
Portraying the role model of the family doctor as a self assured quality professional is paramount in raising the ethos of the discipline in India.
  • The first trip will be in September 2009 (still to be finalised)
  • Other trips may be arranged 2-3 times per year in future
  • The time away would be about 15 days. This includes travel, and two days orientation as a group before dispersal to contact program sites.
  • Visiting consultants will receive allowances to cover return air fare to India, food and accommodation, but they would give their time.

November 2009: "Partnerships in health and development" (Short term medical trips)
 
In December 2008, Owen Lewis will make a trip to India and Nepal to visit the contact program with GPs doing the Distance Diploma of Family Medicine of CMC Vellore. He will also visit Emmanuel Hospital Assocition in Delhi, Nick Simons Institute and Patan Hospital in Kathmandu and BP Koirala Institute of Health Sciences Dharan. HSA is contributing to this trip which will help to clarify how we may be involved in short term teaching trips for GP Education in India and Nepal.
 
For next year, three short term trips are planned. Thank you to all that contributed to participating and reflecting on the 2007 trip to Mvumi in Tanzania.
We have learnt many lessons. Recent talks with Dr John Geater of PRIME, an international network of Christians involved in healthcare education, have seen the commencement of collaboration with HealthServe Australia, seehttp://www.prime-international.org.uk/index.htm
Further talks are planned when John Geater visits Australia in April 2009.
We are also grateful to Dr Sam Molind of the USA group GHO (Global Health Outreach) that have also great experience in this area. GHO is linked to the US CMDAssociation. http://www.cmda.org/

We plan to continue our collaboration with PRIME and utilize a similar approach on our three visits.
They are committed to improving the quality and depth of the medical care especially of patients in poorer situations. We do this in partnership with health care providers to give teaching that equips and empowers them to give excellent clinical care for the whole person.
